Moorings Park Community Health offers a Life Care Management program to assist aging individuals with non-legal aspects of planning for current or future care needs. Life Care Management is a growing sector within the healthcare industry. Our aging life care managers are trained healthcare professional and will be a direct advocates for your loved one. Registered nurses direct the life care managers who will play an important role as an adviser, advocate and support with an elder and their family when the need for extra care and assistance arises.
Members of the National Association of Professional Geriatric Care Managers, our Care Managers can help with:
- Advocacy on the client’s behalf
- Communication with long-distance family
- Medical visit coordination & attendance
- Communication with healthcare professionals
- Household management
- Health & daily living monitoring
- Emergency support services (24-hour Hospital & ER visits)
